If you would like to use email to communicate with Dr. Elizabeth, please download the Email Request Form below, fill it out and return it to the office. You can drop it off in person or mail it in.

Dr. Elizabeth’s email is for quick questions/answers. We will contact you for an appointment if she feels it is necessary to discuss your email in person. Email is not intended to be used for any emergency situations. Dr. Elizabeth makes every effort to check her email daily, but this is not always possible and a response to your email may be delayed.

Important Notes About Provider/Patient Email

• Do not use email for emergencies. Call 911!
• We cannot communicate with you by email until your Email Request Form has been received and approved.
• Be sure to carefully read the Request Form and the Important Information About Provider/Patient Email. Let us know if you have any questions before signing the Email Request Form.
• Email is not secure, so do not include private information in your emails to us.
• We cannot accept prescription requests by email.
• We are not set up to handle email scheduling requests. If you'd like an appointment, please call the office.
